    I, by clicking on the link “Insurance agreement terms and conditions” on the website www.tickets.ru (Skytech Ltd.), which is a subagent and uses the Cherehapa Insurance software for the purchase of the insurance policy, hereby confirm the following:
    1.  I am a capable individual and am eligible to conclude insurance agreements on my own behalf and on behalf of other persons listed by me on the website www.tickets.ru.
    2.  In accordance with subarticle 2, article 940, of the Civil Code of RF, I declare to Skytech Ltd., a Cherehapa Insurance sub-agent, which is an agent of VSK IJSC, my intension to conclude an insurance agreement by filling in the forms of the website www.tickets.ru. The implications hereabove mentioned are made by me in person and are an equivalent to an oral declaration about the conclusion of agreement.
    3.  My personal data and personal data of the insured people is correct and authentic, without aliases, pseudonyms or otherwise distorted information.
    4. All Information provided in the Insurance Agreement Application form is complete and truthful.
    5. I am aware that any invalid or incomplete data provided may be the reason of insurance compensation denial.
    6. I have provided my personal email for the receipt of an electronic copy of the insurance agreement. In the event that insurance agreement is not received, I am entitled to contact the Skytech support to get the insurance agreement duplicate.
    7. I confirm that I am informed that, in accordance with part 1, paragraph 6 of Federal Law as of 06.04.2011 № 63-FZ and paragraph 940 of the Civil Code of RF, the receipt of the electronic insurance agreement by email confirms the conclusion of the insurance agreement between me and Ingosstrakh VSK IJSC under the conditions herein listed and insurance rules applicable to this agreement.
    8. I am entitled to give consent to the processing of personal data on behalf of all persons listed by me on the website www.tickets.ru (Skytech Ltd.), which is a Cherehapa Insurance subagent and uses the Cherehapa Insurance software for the purchase of insurance policy.
    9. In accordance with the Federal Law as of 27.07.2006 № 152-FZ “On Personal Data” (The Law below) I give my consent and consent of all people listed by me on the website www.tickets.ru to process the personal data, including all operations mentioned in subarticle 3, article 3, of The Law hereabove mentioned for the entire period of the Insurance policy and 5 (five) years after its expiration date. The consent is given to Skytech LLC (26A Novorossiyskaya St., Suite 2, St Petersburg, 194156, Russia, TIN 7802720863), Cherehapa Insurance LLC (30 Kozlova St., Moscow, 121357, Russia, TIN 7731421945), and VSK IJSC (4 Ostrovnaya St., Moscow, 121552, Russia, TIN 7710026574). The purpose of the personal data processing is the creation, conclusion and fulfillmen of the Insurance Agreement.
    10. I am informed that the rules of denial of the Insurance Agreement, which I conclude on the website www.tickets.ru, are listed in the Insurance rules, which are an inseverable part of the corresponding insurance agreement.
